Extended Home Care Services
A Registered Nurse (RN) Coordinator will explain the services available, assess your overall needs and the services you are requesting. Together you will determine what kind and how much help you feel you need, and whether you will be paying privately or have insurance to cover the cost.
If needed, the RN Coordinator will help you to complete the paperwork necessary to enroll in the service.
Personal Assistance Services
If you need help with your activities of daily living, a certified Nursing Assistant can help you with personal care such as:
Transferring from bed to chair
Toileting and continence management
Companion and Housekeeping Services
If you find yourself or a loved one in a situation where having someone around to help with housekeeping or general everyday tasks would provide a safety net, you may want the services of a Companion or a Housekeeper.
Depending on whether the primary need is to ensure safety and be available for stand by assistance if needed during bathing, dressing, medication reminders, and activities, or actual housekeeping, we offer a continuum of services to meet your needs such as:
Stand by assistance for personal care support
Accompanying to appointments or activities
Meal planning, shopping, preparation
Changing bed linens
Companion and housekeeping activities can be quite varied and are based on your individual needs and interests, however, we do not perform heavy chores.
Respite services provide a short-term temporary relief to those who are caring for a family member. These services are tailored to your specific personal or home care needs required at the time.
If you require a higher level of care, the private duty services of a Licensed Practical Nurse (LPN) are available. Our LPN’s can help you stay at home and manage your care from as little as medication management to your long term chronic care needs. Additionally, we collaborate with other organizations outside of the home, caring from infants to seniors.
Geriatric Care Management
Life is busy, and we sometimes need someone to help our loved one develop a plan to remain at home, find resources in the community to provide all of the needed supports, make sure they are safe, and be the liaison with the family.
A Registered Nurse (RN) can provide care management to coordinate all of the steps needed to set up a plan and help to implement and monitor its effectiveness.
We can be your point of contact to check on how your loved one is doing and to communicate any additional needs they may have.To learn more or to request services, contact us at 724-463-1102.
